Appointments

New patients are asked to arrive 15 minutes prior to their appointment time to fill out the necessary paper work.

Billing

Insurance companies often apply a copay, coinsurance, and/or a deductible for services rendered in the office.

What to bring to your appointment?

1.Your Insurance Card(s) and Photo Identification
2.All Radiology Reports (MRI, CT, X-ray )
3.List of ALL Medications that you are currently taking, including any over-the-counter medications. Please include the dosage and times per day you take your medications.
4.Name and address of your pharmacy.
5.Name, address, phone and fax number of your Referring Physician
6.Your AUTHORIZATION if you have an insurance plan that requires a referral and/or authorization prior to your visit.

NOTE: You or your primary care physician must fax (954-983-2476) this Authorization to our office prior to your visit. Please note that we will have to reschedule your appointment if we have not received this authorization before your appointment.




Appointment Confirmations / Cancellations / No Shows

As a courtesy, our office will attempt to call you to confirm your appointment two days before your visit. If for any reason you cannot make your appointment please call us at least 24 hours in advance. There will be a $30 fee for missing a scheduled appointment or canceling with less than 24-hours’ notice. Missing a scheduled EEG or EMG test or canceling an EEG or EMG appointment with less than 24-hours’ notice or "NO SHOW" will result in a charge of $250.




Prescriptions and Renewals

Please provide us the name and address of the pharmacy you would like your prescriptions sent to.

All prescriptions and authorizations for renewals should be requested during our normal office hours:
Monday to Friday 9:00 a.m. to 5:00p.m.

Please try not to run out medication and call at the last minute. Refills will be provided within 24 hours. Prescriptions will not be given on after hours or on weekends.

Nerve Conduction Study

A Nerve Conduction Study or NCS is a test commonly performed for the diagnosis or treatment of disease and injuries to the muscles and nerves. In this test, three small electrodes are attached to the skin. These three electrodes records electrical impulses while nerves are stimulated. This process is the repeated for each nerve that needs to be tested.

Electromyography

An Electromyogram or EMG is a diagnostic procedure to assess the health of the nerves and the nerve cells that control them. An EMG is often performed at the same tune as a Nerve Conduction study.

Electrocephalography

An Electrocephalogram or EEG is a painless, non-invasive test performed to detect abnormalities in the activity of the brain. Small flat electrodes are attached to the scalp using a special conducting paste. These electrodes record tiny electrical charges from the activity of the brain cell. Additional test may be performed to measure electrical activity in the brain in response to the stimulation by sight (e.g. flashing lights), sound or touch.

What you need to know for your appointment?

EMG/NCS

1.Do not apply oil or skin lotion on the day of the test.
2.Wear comfortable clothes that can be easily removed, if required.
3.You can take your daily medications as usual.
4.This test can last up to 2 hours so be sure you eat before coming.

EEG

1.Do not use any hair care products such as conditioner or styling products.
2.Bring a hair brush.
3.You will be asked to remove any jewelry, hair pins eyeglasses, hearing aids or other metal objects that may interfere with the procedure.

**Please be on time for your appointment. If you need to reschedule your testing appointment for any reason, please call the office at least 72 hours in advance. A “No Show’ fee will be charged if the appointment is cancelled with a less than 24-hour notice.

Health Insurance

We accept most insurance plans. Payment plans can be arranged for cash paying patients for your doctor visit and most procedures.

Monday to Friday 9:00 a.m. to 5:00 p.m. If prepayment is not received by the office, it will be collected when you check-in for your appointment.

We accept Visa, MasterCard, American Express, Discover as well as cash and personal check.

NOTE: A returned check fee of $35 will be levied by this office for any check returned for insufficient funds. Your bank may also charge you an additional returned check fee.
